According to a recent press release on NuHealth.net, 3 out of 4 NuHealth System Family Health Centers received the highest designation of Level 3 Patient Centered Medical Homes. Level 3 is the highest designation under guidelines by the National Committee for Quality Assurance and the NYS Department of Helath.
NuHealth’s Family Health Centers in Roosevelt, Elmont and NewCassel received the designation, and the fourth location in Hempstead is expected to receive Level 3 in the near future. Currently, Hempstead is a Level 2 Patient Centered Medical Homes.A Patient Centered Medical Home is defined as “a model of care where each patient has an ongoing relationship with a personal physician who leads a team that takes collective responsibility for patient care,” (NuHealth.net). The AHRQ defines a medical home as “not simply a place but a model of primary care that delivers care that is: patient centered, comprehensive, coordinated, accessible and continuously improved through a systems based approach to quality and safety,” (NuHealth.net). This is a distinguished honor for NuHelath System to receive.
